Applying for the Training Certificate
Below are the procedure for fellows applying to APAGE headquarters for a training certificate.
The steps are as follows:
- The applicant should send an email to APAGE headquarter at apage.membership@gmail.com, specifying the hospital name of the APAGE accredited training center, training period, mentor, and training type (Clinical hands-on or Observership hands-off).
*Please note that the mentor must be a surgeon accredited by APAGE.
- APAGE headquarter will then contact the Department of Education of the training center to confirm and verify the applicant’s information, including the training period, mentor, and type of training (Clinical hands-on or Observership hands-off).
- The applicant will be asked to submit the following documents at least 1.5 months prior to the end date of the training:
- CV
- Application form
- Copy of Diploma (Bachelor, Master, or Ph.D., translated to English and notarized if required)
- Copy of medical practice license (in English)
- Copy of passport
- Formal color photo (2x2 inches)
- Assessment form (if your training center has a similar assessment form, please forward it to us).
- Applicant must register as a 10-year APAGE member on the official APAGE website.
- Once the above documents are received and the applicant has completed their APAGE 10-year membership registration, we will send the training certificate immediately.
